One busy day || April

“Yes, you’re right” said Eveny “There are indeed extra symptoms to migraines. And the most frequent when they are caused by damaged vessels are hallucinations. But…” She made the chair turn towards April “But the most interesting part of this is that the patient haven’t said anything about any hallucination, of any kind.” She stood up, smiling brightly. “So, I think it’s time for a little visit !”

She knew some doctors loathed to do visits, finding them boring and useless — but definitely not Eveny. It was almost what she liked most about her job, that human contact. Besides she was intimately convinced that visits could help a great deal with treating the patient… and sometimes the diagnosis.

For when she entered the patient room, the young woman wasn’t exactly lying still in bed. She was screaming, running through the room, holding her head between her hands, with the nurses trying to put her back in her bed. “Lay her down !” said Eveny hastily. “Take off the cushions ! Quick !” As soon as the woman had been lain on her back, her head on the same level as the rest of her body, the pain went away, and she stopped screaming.
“We’re missing the point” said Eveny, turning to April.

Everything happened so fast. At first they were just having a conversation and then there the patient went, screaming in agony. April did as Eveny said, as quicky as possible, and then pushed a piece of hair behind her ear. Everything seemed so peaceful again; the patient didn’t scream and she seemed absolutely fine. April nodded her head firmly and bit her lower lip, confused. What was going on?

“Yes, absolutely. But what’s going on? Was this a new thing? Ever happened before?” April asked, frowning a little bit. It was weird and sort of unexpected. Things like that happened but if it was a new symptom, there must have been a trigger to cause all that pain the patient had just experienced. But what was that trigger? They had to figure out something, quick.

“She seems to be fine when laying down, that’s for sure. What would cause that kind of pain whilst standing?” she wondered and walked around a little bit, chewing her lip and sometimes looking between the patient and Eveny. She was a bit confused.

“Let’s go out, shall we ?” Eveny said before exiting the room, followed by April. It was indeed very, very strange — she had never witnessed such symptoms for migraines before, and she doubted it was still about headache…

“I think I know what’s wrong” the neurologist said after a long reflexion. She knew something that could cause such pain, and then disappear when lying down. She had already seen that sort of cases in other patients, but never in someone who had complained about migraines before. “Her cranium is lacking cerebrospinal fluid. When she’s lying down, it balances the whole thing again and she doesn’t suffer anymore.”

She stood, wondering, almost for herself “But why now ? And what about the damaged artery on the scan ? Or could it be that…” She wasn’t sure of herself, even a little afraid of the meaning of the whole thing if she was right. “Her cerebral aqueduct may have been damaged because of a shock of any kind, causing her pain, and now it’s leaky and she’s lacking the fluid. And then the artery is…” she stopped for a moment, to be completely sure of herself. But how could she be wrong, when it appeared so clearly on the scan. “Yes, it is an aneurysm.”

Eveny suddenly became grave. It was too deep in the brain to be operated on. There was nothing she could do about that. She would repair the cerebal aqueduct and then tell her patient that she could die at any moment. Eveny was used to losing people while working, but every time, it still was hard for her to admit that she couldn’t always save everybody.
